Clinical support after you leave hospital
After an admission at The Wesley Hospital, patients may benefit from ongoing professional and personalised healthcare provided in the comfort and privacy of your own home. Depending on your eligibility, we provide two options for at-home care delivered in partnership with BlueCare, a trusted community and home care provider that is also part of the UnitingCare family.
- Hospital in the Home (HITH): Is available for patients who can safely receive ongoing care at home, while remaining an admitted patient of the hospital. Until you can be discharged, highly qualified BlueCare nurses and your treating specialist deliver your personalised treatment plan at home, including a daily visit from BlueCare, and support when you need it 24 hours a day.
- Discharged Post-Acute Care: Is available for patients who may benefit from additional clinical support after discharge from hospital. Your care will be determined in your personalised care plan as you transition home from hospital, and will be provided by highly qualified BlueCare nurses at home in accordance with your treating specialist. You’ll be able to access care when you need it 24 hours a day.
Please note: Eligibility may vary based on your private health insurance cover, as not all providers include this service.
- Your medical team determines your eligibility to receive treatment safely at home.
- The Hospital in the Home / Discharged Post-Acute Care team liaises with the hospital medical team and private health insurer to determine your eligibility, treatment and recovery plan.
- Arrangements are made for you to transfer home and continue your treatment.
- Your treatment is delivered in line with your medical plan, with your progress monitored carefully by the BlueCare and hospital team until you no longer require care.
Receiving care at home can greatly decrease the amount of disruption your clinical treatment has on your life. With the Hospital in the Home and Discharged Post-Acute Care services, you receive hospital-quality care within the comfort and privacy of your own home.
These services are suitable for patients with medical conditions that are safe and appropriate to treat at home. Eligibility will depend on your individual treatment requirements and private health insurance cover. You may be eligible to receive Hospital in the Home or Discharged Post-Acute Care services if you have one of the following conditions:
- Skin inflammations or infections.
- Blood clots.
- Pneumonia or other chest infections.
- Kidney or urinary tract infections.
- Mastitis.
- Diverticulitis.
- Appendicitis.
Even if you don’t have one of these conditions, you can still talk with your doctor or nurse to check whether you may be eligible.
